By sending this form you agree to have read and accepted the Terms and Conditions of the letting of Wern Cottage as previously sent to you in the booking conditions policy document and confirm that you are over 21 years of age.

You agree to leave the property in a clean and tidy condition, and be responsible for any and all damages or breakages howsoever caused.
